How can I become a member?

Becoming a member is easy.

1. Create an account
2. Place an order
3. Register for a medical document (the form will be emailed to you)

Additionally, you can transfer a valid medical document from another provider, and you'll receive $50 off your first order.

Once you have completed the registration process, a practitioner will review your document within 1 business day. After you have been approved, you will receive an email confirmation, and your order will ship within 1 business day.
